POS 22: On-Campus Outpatient Hospital Place of Service Explained

Jupiter RCM article cover: POS 22: On-Campus Outpatient Hospital Place of Service Explained

POS 22 is the place of service code for an on-campus outpatient hospital department. When a provider sees a patient there, the provider bills a professional claim with POS 22, which Medicare pays at the lower facility rate, and the hospital bills its own facility claim for the space, staff and equipment.

What is POS 22?

CMS names POS 22 “On Campus-Outpatient Hospital” and defines it as “A portion of a hospital’s main campus which provides diagnostic, therapeutic (both surgical and nonsurgical), and rehabilitation services to sick or injured persons who do not require hospitalization or institutionalization.” Departments away from the main campus use POS 19, Off Campus-Outpatient Hospital.

How a POS 22 visit is billed

  • Professional claim: the provider bills the CMS-1500 or 837P with POS 22. Medicare pays the facility rate, which leaves out practice overhead.
  • Facility claim: the hospital bills the UB-04 or 837I for its outpatient services.
  • Patient cost: the patient may owe cost-sharing on both claims, which is why hospital outpatient visits often cost patients more than office visits.

POS 22 vs. POS 11 vs. POS 19

  • POS 22: provider-based hospital department on the main campus.
  • POS 19: provider-based hospital department off the main campus.
  • POS 11: a freestanding office, even if a hospital owns the practice, when it is not a provider-based department.

Common POS 22 mistakes

  • Billing POS 11 from a provider-based department, which overstates the professional payment and is a common audit finding.
  • Billing POS 22 from an off-campus department that should be POS 19.
  • Billing POS 22 from a freestanding office after a practice acquisition, before the department is actually provider-based.

Frequently asked questions

Why is POS 22 paid less than POS 11?

Because the hospital is paid separately for the overhead through its facility claim. The professional payment covers only the provider’s work.

How do I know if our clinic is provider-based?

Ask the hospital’s compliance or billing office. Provider-based status is a formal designation with Medicare requirements, not just ownership.
