Eligibility verification

Insurance Eligibility Verification Services

We run patient eligibility verification before each visit, so the front desk knows what is covered, what the patient owes and whether an authorization is needed.

What we confirm

  • Active coverage and plan details
  • In-network or out-of-network status
  • Copay, coinsurance and deductible
  • Prior authorization and referral needs
  • Primary and secondary plan order

What is insurance eligibility verification?

Insurance eligibility verification is the check a practice runs before a visit to confirm that a patient’s coverage is active and that the planned service is covered. A full check goes further than active or inactive. It confirms the plan and payer, whether you are in network, the copay, coinsurance and remaining deductible, and whether the service needs a prior authorization or referral. It also confirms which plan is primary when a patient has more than one. Most checks run through the payer’s electronic 270/271 eligibility transaction or its provider portal. When information is missing or unclear, we call the payer. Jupiter RCM verifies upcoming appointments ahead of time and flags coverage problems before the visit, so the front desk can update insurance details or collect the patient’s share at check-in. That matters because denials for terminated coverage, an unidentified patient or a missing authorization are among the easiest to prevent and some of the hardest to recover later.

The problem

What happens when coverage is not checked

Eligibility problems rarely show up at the front desk. They show up weeks later as denials, rebills and patient balances that are hard to collect.

Terminated coverage

The patient changed plans or lost coverage, and the claim goes to a payer that will not pay it.

Wrong member details

A mistyped member ID, name or date of birth means the payer cannot find the patient.

Missing authorization

A service that needed prior approval is performed without it and denied.

Surprise balances

Patients learn what they owe only when a statement arrives, which makes collection harder.

How we handle it

What our eligibility checks cover

We check scheduled patients ahead of their visits and record the results where your team can see them.

  • Active coverage and effective dates
  • Plan type and payer details
  • Network status for the rendering provider
  • Copay, coinsurance and remaining deductible
  • Visit limits and service-specific benefits
  • Prior authorization and referral requirements
  • Coordination of benefits and plan order
  • Medicare, Medicaid and commercial plans
  • Flags for the front desk before check-in

Denials it helps prevent

Common eligibility-related denial reasons

These claim adjustment reason codes (CARCs) often trace back to a coverage check that was skipped or incomplete.

FAQ

Insurance Eligibility Verification Services: common questions

How far ahead do you verify eligibility?

Usually a few days before the appointment, with a recheck when a plan is known to change often, such as Medicaid. We agree on the timing with you based on how your schedule is booked.

Do you handle prior authorizations?

Yes. Every eligibility check notes whether a service needs authorization. Our prior authorization service then prepares, submits and tracks the request until the payer decides.

Can eligibility verification be used on its own?

Yes. Some practices use it as a standalone service, while others get it as part of full revenue cycle management.
