POS 10 is the place of service code for telehealth when the patient is at home. It was created in 2022 to separate home telehealth from telehealth at other sites (POS 02). For Medicare, services billed with POS 10 are paid at the non-facility rate, the same rate as an office visit.
What is POS 10?
CMS names POS 10 “Telehealth Provided in Patient’s Home” and defines it as “The location where health services and health related services are provided or received, through telecommunication technology. Patient is located in their home (which is a location other than a hospital or other facility where the patient receives care in a private residence) when receiving health services or health related services through telecommunication technology.” It took effect January 1, 2022, and became available to Medicare on April 1, 2022.
When to use POS 10
- The patient joins from their own home or another private residence.
- Behavioral health sessions by video, or by audio only where the payer allows it.
- Follow-up visits and other services on the payer’s telehealth list, when the patient is at home.
Medicare rules for home telehealth
Medicare permanently allows mental and behavioral health telehealth with the patient at home. For most other services, home telehealth has depended on temporary extensions from Congress. As of October 2026, those flexibilities run through December 31, 2027, and the in-person visit requirement for most mental health telehealth is delayed until January 1, 2028 (Medical Economics summary (opens in a new tab)). Check CMS telehealth guidance (opens in a new tab) before relying on these dates, because they have changed several times.
Common POS 10 mistakes
- Using POS 02 for a patient at home, which Medicare pays at the lower facility rate.
- Using POS 10 when the patient was at a clinic, workplace or school.
- Missing the payer’s modifier, such as 93 for audio-only Medicare services or 95 for plans that require it.
- Billing home telehealth to a commercial plan whose contract does not cover it.
