“Entity’s Tax ID” Claim Rejection: How to Fix It

Jupiter RCM article cover: “Entity’s Tax ID” Claim Rejection: How to Fix It

Status code 128 means the tax ID on the claim is missing, invalid, or does not match what the payer has on file for the provider named by the entity code, usually the billing provider (85). Payers link each NPI to a tax ID through enrollment, so both have to match.

What does status code 128 mean?

X12 claim status code 128 reads: “Entity’s tax id. Usage: This code requires use of an Entity Code.” It is published in the X12 Claim Status Codes list (opens in a new tab). The entity code tells you whose tax ID to check.

On the raw 277CA it appears in the STC segment, for example STC*A7:128:85: the claim status category, then the status code, then the entity code when one is required. A rejected claim never entered the payer’s claims system, so there is nothing to appeal: you fix the data and send the claim again.

Common causes

CauseWhat to check
Wrong EIN entered in the billing provider setupIRS documents and the payer enrollment
Practice changed ownership or tax ID without updating payer enrollmentEnrollment records with each payer
Individual’s SSN sent where the group EIN belongs, or the reverseBilling provider type
NPI and tax ID combination not enrolled togetherPayer enrollment by NPI and tax ID

How to fix it

  1. Use the entity code to identify which provider’s tax ID failed.
  2. Compare the tax ID on the claim with the IRS records and the payer’s enrollment for that NPI.
  3. If enrollment is out of date, update it with the payer before resubmitting.
  4. Correct the provider setup and resubmit as a new claim.

How to prevent it

  • Update every payer before a tax ID or ownership change takes effect.
  • Store the NPI and tax ID pairing for each payer in one place.
  • Watch the first claims after any change for rejections.

Frequently asked questions

We changed our tax ID. Why are only some payers rejecting?

Each payer updates enrollment separately. Payers that have not processed your change will reject until they do.

Can I fix status 128 by changing the claim only?

Only if the claim had a typo. If enrollment is wrong, the payer will keep rejecting until it is updated.
