CPT 90785 is an add-on code for interactive complexity. It is billed on top of a psychiatric evaluation or psychotherapy code when a specific communication factor made the session harder to deliver, such as a disruptive family member, a mandated abuse report, or the need for play equipment or other devices. It is never billed on its own.
CPT 90785 description
90785 reports interactive complexity: communication factors during the visit that complicate delivery of the primary service. It does not depend on how complicated the diagnosis is. The American Psychiatric Association guide to interactive complexity (opens in a new tab) explains the factors in detail.
The four factors that qualify
At least one of these must be present and documented:
- Maladaptive communication among participants, such as high anxiety, high reactivity, repeated questions or disagreement, that complicates care.
- Caregiver emotions or behavior that interfere with the caregiver’s understanding and ability to help carry out the treatment plan.
- A sentinel event and mandated report, such as abuse or neglect reported to a state agency, with discussion of the event or report with the patient and other participants.
- Play equipment, other physical devices, an interpreter or a translator used to overcome barriers with a patient who is not fluent in the clinician’s language or lacks the expressive or receptive language skills to communicate.

Common 90785 mistakes
- Interpretation alone: CMS guidance says 90785 generally should not be billed solely for translation or interpretation services.
- Billing it every session: interactive complexity depends on what happened in that session, not on the patient’s diagnosis or age.
- No documentation: the note must name the factor and how it affected the session.
- Billing it alone or with crisis codes: both are denied.
Frequently asked questions
Does 90785 add time to the session?
No. It has no time requirement of its own and does not change which psychotherapy code the session time supports.
Can 90785 be billed for a child’s session?
Only when a qualifying factor is present, such as play equipment needed to communicate or a caregiver whose behavior interferes with treatment. Age alone does not qualify.
