CPT 90834: How to Bill 45-Minute Psychotherapy

Jupiter RCM article cover: CPT 90834: How to Bill 45-Minute Psychotherapy

CPT 90834 is the code for 45 minutes of individual psychotherapy, reported when the session lasts 38 to 52 minutes. It is the right code for the classic 45 to 50-minute therapy session, which is why it is billed more often than any other psychotherapy code.

CPT 90834 description

90834 is psychotherapy, 45 minutes, with the patient (family members may take part). Under CPT time rules it covers 38 to 52 minutes of psychotherapy time. Below 38 minutes the session is a 90832; at 53 minutes or more it is a 90837. See the American Psychiatric Association CPT overview (opens in a new tab) for the full set of psychotherapy codes.

When 90834 is the right code

  • A standard 45 or 50-minute session: both fall inside 38 to 52 minutes.
  • A planned hour that ran short, ending before 53 minutes.
  • Telehealth sessions of the same length, billed with the payer’s telehealth place of service and modifiers.

Common 90834 billing mistakes

  • Billing by the schedule, not the session: a 50-minute appointment slot that ran 35 minutes is a 90832.
  • Missing time: no start and stop times or total time in the note.
  • Wrong code with an E/M visit: prescribers who also bill an E/M visit use add-on +90836 instead.
  • Credentialing gaps: the clinician is not enrolled with the plan or the behavioral health carve-out that manages the benefit.

Frequently asked questions

Is a 50-minute session 90834 or 90837?

90834. The 90837 code starts at 53 minutes. Our 90837 vs. 90834 guide compares the two.

Can 90834 be billed with 90791 on the same day?

Not by the same provider. The evaluation and psychotherapy are generally billed on separate dates.

Does 90834 need prior authorization?

It depends on the plan. Some require authorization after a set number of visits, so check benefits before treatment continues.
